A while back I asked for samples of checklists from the members here. I got several people who helped by emailing stuff to me. Since then I have used ideas from, and parts from these lists. I have also added my own twists.

Anyway, here is a link to a Word doc checklist I now use to help me gather information. It's a living document and can be edited and I do edit from time to time. So many have helped me so I decided to put this up. Maybe, just maybe it will help someone in return.

Feel free to ask questions, make suggestions or comments about ways you think might improve the list.

Is it in line with ASHI standards or any standards in particular

It's pretty much in line with ASHI, NAHI and my State of MD SOP. It might need a tad of tweaking. I meant to make the mandatory things in blue but again, I may need to adjust it just a bit.

Very nice! I especially liked the notes you included (ie wire ga/amps). I can usually remember that kinda stuff but not the citations requiring it, I'll definately be adding notes like that to mine.
