I called out some loose carpet in a stairway last week. The carpet was cut in individual pieces, one for each rise/run. It was held down with tack strips only and was popping loose in several places.

I was just wondering, shouldn't the carpet for the stairs be one continuous piece?

Not my experience at all. With the exception of some much older ( and worn out) jobs, the tread, nose and riser are all covered. Its usually not done in one piece, but it ends up looking that way.
