The pan leaks and the framing is mush.

My two bits: The sill used to frame a curb was installed all the way around the shower and wasn't even with studs.

Some combination of crap caused by a failed pan liner. Maybe moving along a substrate seam.

Regardless, it looks like you got a septic tank in there under the tile.
