230.7 Conductors other than service conductors shall not be installed in the same service raceway or service cable.

Exception #1: Grounding conductors and bonding jumpers.

Exception #2: Load management control conductors having overcurrent protection.

I don't think they are coax but I didn't look closely. My attention was overwhelmed by a couple of wildly overcrowded conduits and what looked like over heated wires.

The interior surfaces of the enclosure do indeed look like arcing has taken place plus lots of wire nuts connecting red conductors to black ones.

Could it be that someone installed a new panel in the existing enclosure?

The bottom right 50 amp generic breaker QO knockoff. Acceptable or not?

If the breaker is "classified" for use in that panel (it'll have the word "classified" printed on it somewhere) then there's no question that it's ok.

Even if it's not, if the breaker fits in its position snugly without pushing anything else out of the way and if the cover fits properly over it, I don't say anything about it.

The only time I call out off-brand breakers is when they don't fit properly.
